HCBS-RME
A Recovery Management Entity (RME) is a specialized organization that provides care coordination and recovery support services for individuals with mental health and/or substance use disorders. The core offerings of an RME typically include:
1. Recovery Planning & Coordination
- Develops person-centered recovery plans tailored to the individual’s strengths, needs, and goals.
- Coordinates care across multiple systems (behavioral health, physical health, housing, employment, etc.).
2.Ongoing Recovery Support
- Provides coaching, mentoring, and support to help individuals maintain recovery.
- Assists clients in accessing community resources, such as transportation, housing, and peer support.
3. Service Navigation
- Helps individuals engage or re-engage with behavioral health services, primary care, social services, and community-based supports.
4. Monitoring & Advocacy
- Monitors client progress and advocates for services that promote recovery and stability.
- Ensures timely service delivery and helps resolve barriers to care.
5. Collaboration with Providers
- Works in partnership with local mental health authorities, substance use providers, Medicaid managed care organizations (MCOs), and other stakeholders.
