Targeted Case Management (TCM)
Targeted Case Management (TCM) is a Medicaid-funded service that helps individuals with serious mental illness, chronic health conditions, developmental disabilities, or other qualifying needs to access and coordinate essential care and support services. The goal is to improve overall well-being, stability, and independence in the community.
This service is often delivered by a team of case managers or qualified mental health professionals, and must adhere to state and Medicaid regulations.
1. Assessment & Planning
- Conduct comprehensive assessments of clients’ medical, mental health, social, and functional needs.
- Develop individualized care or service plans with measurable goals based on client strengths and needs.
2. Service Coordination
Link clients to appropriate services, such as:
- Primary and behavioral healthcare
- Housing and food assistance
- Educational and vocational services
- Transportation
- Legal or financial aid
- Coordinate among multiple service providers to ensure continuity of care.
3. Monitoring & Advocacy
- Monitor service delivery and client progress.
- Advocate for clients’ rights and access to services.
- Make timely updates to care plans based on changes in the client’s needs.
4. Crisis Intervention & Support
- Identify and address crisis situations to reduce risk of hospitalization, homelessness, or institutionalization.
- Provide support to help clients remain in the least restrictive setting possible.
5. Documentation & Compliance
- Maintain detailed service notes and progress documentation for compliance with Medicaid and Managed Care Organizations (MCOs).
